Welcome to our practice policies. Please read through the policies before visiting our office. 

FEES AND PAYMENT In an effort to uphold the therapeutic freedom and integrity of our practice of medicine, we do not accept insurance plans at CMA. This enables us to work outside the constraints and requirements of health insurance carriers and ensures both the time and freedom to provide the comprehensive care needed to address your health concerns. Fees are due at the time of service. Methods of payment include cash, check,(and for Dr. Johnson, credit card). We provide coded receipts that can be submitted to your insurance company. Remedies and visits are covered under most Health Saving Accounts.

Communication Policy Please note that our communication policy has recently changed. We communicate exclusively via phone call and require our patients to listen to the messages that we leave before calling us back. Please read our full policy here.

APPOINTMENTS Appointments for the physicians and massage therapist can be made by calling CMA directly at 518-721-8417. For appointments with Dr. DeRosa-Lazare, Dr. Sims-O’Neil, and our therapists, please talk with Donna. For appointments with Dr. Johnson, please talk with Liz.

CANCELLATION POLICY Cancellations may be made by phone. We appreciate being informed at least 48 hours before your appointment if you need to cancel so that we can give that time to another patient in need. Please note we have a 24 hour cancellation policy. If you cancel less than 24 hrs before your appointment or miss your appointment you may be charged a fee. For more information please ask our front desk.

CELL PHONES Please silence cell phones and refrain from their use while in the waiting and exam rooms. 

FRAGRANCES Please do not wear fragrances to the office. Many people are sensitive and/or allergic to environmental pollutants such as perfume, hair sprays etc. 

CHILDREN We are a child friendly office. There are plenty of toys and books for your child to enjoy while here. We can not offer child supervision and expect that children will be supervised and kept in the waiting or exam rooms with the supervising adult. 

PHONE CALLS Dr. DeRosa-Lazare is available by phone after office hours for established patients only. These calls are for acute problems (new onset issue in the last 12 hours) only. Patients who text or leave a message for a non acute issue will have to call the office the next work day for a response. A fee will be charged for scheduled and acute calls lasting more than 5 minutes. 

Dr. Johnson is available for home visits, phone consults, and HIPPA-secure video conferences on a pre-arranged basis.

HOME VISITS Are available for those who are too ill to come into the office and for newborn exams. Fees are determined by location and length of visit.

FINANCIAL HARDSHIPS If you have financial hardships please speak to Donna or Liz as needed.
